** Starred Payors and/ or HMO plans (Health Maintenance Organization), require oversight by your chosen Primary Care Provider (PCP). As a specialty medical practice, we require an authorization or referral from your insurance plan for you to be seen. Please contact your PCP and/ or insurance to ensure that you have an active referral on file, or you will need to be rescheduled.
NO SURPRISE ACT (NSA)
In January 2022, the No Surprises Act (NSA) was put into federal law to assist patients regarding their patient responsibility. As part of our policy, we share pricing prior to your visit. Our office is classified as a specialty healthcare and a new patient visit begins at $250 and an established visit starts at $150, not including labs.
We do not always know what will transpire during your appointment, but we guarantee that we will explain all costs up front prior to any additional service. If at any time you have any questions about the cost of your services, please contact us at 346-257-4299 option 5 for a GOOD FAITH ESTIMATE (GFE).
Please note, unfortunately we do not have access to your laboratory benefits through your insurance. This contract is between Quest or LabCorp and your third-party payor. If you have questions, please contact your insurance prior to your visit regarding lab services.
